Manage patient communication across telephone and electronic health record systems, performing clinical triage, symptom assessment, scheduling and follow-up coordination
Coordinate multidisciplinary care across physicians, advanced practice providers, patients, caregivers, pharmacies, specialists, and community resources to ensure continuity of care
Process provider-directed clinical workflows, including medication refills, laboratory and diagnostic orders, referrals, and external medical record retrieval and review
Complete patient documentation and required forms accurately within the electronic health record (EHR) and facilitate other electronic workflows
Provide patient and family education related to treatment plans, medications, preventive care, and chronic disease management, with emphasis on complex geriatric populations
Serve as a clinical resource and support to other nursing staff regarding triage decisions, prioritization, and care coordination processes
Maintain compliance with HIPAA and organizational policies, participate in quality improvement initiatives, and maintain required clinical competencies and certifications
Perform other duties as assigned
Requirements
Graduate of an accredited nursing program with two years of nursing experience
Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with the Compact Act
Basic Life Support Certification or obtains certification within one month of employment
