Provide remote operational support to Care Managers across centers by backfilling vacancies as needed.
Conduct proactive outreach to patients to complete missed care plans.
Identify patient care requirements by establishing personal rapport with patients.
Leverage patient data from dashboards and reports to identify trends and improve patient engagement.
Accurately document patient care services.
Address patient needs related to opioid use disorder.
Establish individualized patient care goals in collaboration with patients.
Schedule and reschedule patient appointments.
Monitor patients’ clinical status.
Assist physicians and clinicians with patient onboarding and orientation.
Maintain patient information confidentiality and privacy.

At minimum, High School Diploma or GED required.
Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Knowledge of compliant documentation practices.
Lived Addiction experience is a plus.
Can-do attitude and willingness to think beyond the status quo.
