Role Overview
- Provide advanced clinical support to patients within the Aging and Dementia Division of Neurology
- Facilitate patient care on an outpatient setting
- Provide order and prescription entry into EMR
- Offer patient education on new medications/therapy management
- Manage high-profile therapies and ensure therapy adherence
- Collaborate with a multidisciplinary team to improve patient outcomes
- Triage calls and assess patients over the phone.
- Coverage of other staff within the same nurse coordinator role when necessary
Requirements
- Associate Degree In Nursing or Diploma In Registered Nursing
- Current RN licensure in the state of Missouri and/or Illinois
- Basic Life Support Certification or obtains certification within one month of employment
- Graduate of an accredited nursing program with two years of nursing experience
- Strong patient care skills
