RN Clinical Nurse Coordinator – Cardiology

Role Overview

• Serves as a vital liaison between patients and cardiology providers in a busy outpatient practice
• Supports 2–4 physicians or advanced practice providers
• Combines direct patient support, telephone triage, symptom assessment, patient education, care coordination, and follow-up management
• Provides direct patient care, including assessment, planning, implementation and evaluation of patients
• Completes health history and physical examination on patients
• Coordinates patient care, orders diagnostic tests, conveys findings to physician, attends inpatient rounds, coordinates patient appointments and assists with referrals to outside sources
• Acts as liaison and team leader by facilitating communication and coordination of care between patients, families and health team members

Requirements

• Graduate of an accredited nursing program with two years of nursing experience
• Current RN licensure in the state of Missouri and/or Illinois, depending on location
• Basic Life Support Certification or obtains certification within one month of employment
