Role Overview
Provide comprehensive assessment of social, emotional, and practical needs to patients and families in outpatient PC clinic, private residences, and care facilities.
Introduce and educate regarding Palliative Care and Hospice services.
Conduct independent psychosocial assessment/evaluations and provide 1:1 psychotherapy.
Participate in professional team reviews of patient care and cases.
Provide casework and group work for patients and families.
Identify communication resources and assist patients and families in connection with resources and advance care planning.
Responsible for continuing professional development to enhance practice.
Requirements
MSW from a CSWE accredited School of Social Work.
Licensed Clinical Social Work license required.
Completion of American Cancer Society Leadership in Oncology Navigation / PIN training and credentialing within one year of hire.
At least two years of professional social work experience, preferably in a health care setting.
