Community Health Worker – Intern

Role Overview

• Participates in a structured six (6) month rotational program across various functions of the Community Health and Community Benefits team.
• Shadows, supports, and learns from certified Community Health Workers (CHWs).
• Conducts intake, needs & eligibility assessments, and referrals of patients to appropriate community resources under staff supervision.
• Rotates working locations across community health programs at Memorial Hermann locations across the greater Houston area.
• Completes a project and/or presentation at end of rotation that demonstrates knowledge obtained during experience.

Requirements

• High school diploma or GED preferred.
• Community Health Worker Certification preferred.
• Strong coordination and organizational skills.
• Ability to work collaboratively with other professionals in fast-paced, service-oriented environment.
• Ability to navigate ambiguity or flexibility in different work environments.
