Provide compassionate and skilled oncology expertise spanning the entire cancer care continuum; including diagnosis, treatment, survivorship, and end of life care.
Assist patients, families and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care.
Facilitate patient care coordination with interdisciplinary teams within internal departments and external service lines.
Provide education to patients and caregivers, supporting compliance with appointments and treatments.
Work closely with external services to ensure seamless coordination of outpatient care, including chemotherapy, infusion therapy, radiation, and other treatments.
Facilitate the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement.
Promote a safe working environment by complying with department and institutional safety policies.
Requirements
Associate’s degree in nursing, Diploma RN, Bachelor’s Degree in Nursing or Master’s degree in Nursing plus 3 years Registered Nurse experience
Graduate of an accredited RN nursing program
Valid Registered Nursing License
Basic skills with Microsoft programs, Word, Excel, PowerPoint
Excellent communication skills
Strong work ethics and accountability.
Preferred Qualifications:
Bachelor’s Degree or higher
Minimum of 1 year of relevant Registered Nursing experience in Oncology or Minimum of 5 years in patient navigation/coordination
Current BLS National Certification – AONN or OCN or Oncology related certification
Knowledgeable with EPIC EMR
