About this role
Role Overview
- Provides case management services and coordinates programmatic designation for Harc Day Programs individuals
- Primary liaison between Harc and funding agencies, parents, and residential providers
- Writes timely, comprehensive reports and attends scheduled individual planned meetings
- Directly supervises direct support staff during program functions
- Provides direct care support when needed
- Responsible for monitoring and coordinating the Individual Plan
Requirements
- Knowledge typically associated with a Bachelor's degree in a human service
- Experience in Special Education, Counseling and/or behavioral programming required
- At least one-year minimum experience
- Competence in Microsoft Word and Excel for Windows
- Must have a valid Driver's License
- Must have reliable transportation
