Community SOC Family Partner

Role Overview

• Educating parents/caregivers about navigating child-serving systems
• Facilitating access to resources for parents/caregivers
• Working alongside System of Care Coordinator in assigned counties
• Participating in System of Care planning and implementation
• Providing education and support about rights and responsibilities
• Serving on multidisciplinary care teams
• Supporting completion of discovery activities
• Assisting members and families in choosing service providers
• Helping members direct the planning process as desired

Requirements

• High School Diploma or GED
• Minimum of four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or State funded BH services
• Must have a valid driver’s license
• Must reside within one of the following North Carolina counties: Anson, Montgomery, Moore, or Richmond
• Must be able to travel within catchment as required
• Two (2) years paid experience working directly with individuals with BH/MH or IDD conditions (preferred)
• Paid experience related to the counties in which they are applying (preferred)
• Certified Peer Support Specialist (preferred)
