Enhances continuity of patient care by providing collaboration between hospital, physicians, hospice/home care, and caregivers
Works with case management, social workers, and others on health care team to transition patient to post-acute care setting
Responsible for building and maintaining partnerships that enhance post-acute care referrals to Alliance
Collaborates to develop and execute market strategies, improve service offerings, and implement initiatives that support improved patient care and growth in division sales
Ensures documentation is complete by reviewing patient orders, verifying insurance information, gathering clinical data and determining patient care needs
Facilitates the transfer of patients from hospital to home care/hospice services and consults with nursing, social services and other support services
