Home Health RN Case Manager

Role Overview

• Provides direct patient care in patients’ homes.
• Manages own patient caseload while completing skilled nursing visits, assessments, patient education, care coordination, and documentation.
• Collaborates closely with LPNs and an interdisciplinary team to ensure high-quality patient outcomes.
• Utilizes independent nursing judgment when integrating health care.
• Monitors, evaluates and trends patient/resident responses utilizing structured care methodologies.
• Establishes priorities and demonstrates strong organizational skills.

Requirements

• Bachelor's degree in Nursing strongly preferred.
• Graduate from an accredited nursing program.
• Minimum of two years’ clinical experience preferred.
• Currently holds an unencumbered RN license.
• Requires valid driver's license depending on location.
• Obtains and maintains required department specific competencies and certifications.
