About this role

This is a field-based home health nursing role providing direct patient care in patients’ homes.
Manage your own patient caseload while completing skilled nursing visits, assessments, patient education, care coordination, and documentation in the home setting.
Work independently in the field while receiving strong support from experienced clinical leadership.
Collaborate closely with an interdisciplinary team to ensure high-quality patient outcomes and coordinated care throughout the episode.
Build meaningful, long-term relationships with patients through consistent caseloads and continuity of care.
Enjoy flexibility and autonomy in structuring your day while serving patients throughout the community.
Experience a balance of hands-on patient care, clinical decision-making, and care coordination in a highly rewarding environment.

Bachelor's degree in Nursing strongly preferred
Graduate from an accredited nursing program
Minimum of two years’ clinical experience preferred
Currently holds an unencumbered RN license with the State Board of Nursing
Obtain and maintain required department specific competencies and certifications
Depending on location, a valid drivers license may be required
