Clinical Pharmacist Advisor, Product & Workflow Design (Part-Time, Path to Full-Time)

Valore Health is building software to help community pharmacists deliver chronic care in partnership with primary care physicians. The Clinical Pharmacist Advisor will guide product and workflow design so the platform reflects real-world pharmacy operations, clinical practice, and documentation needs.

Responsibilities:

• Map the real workflow. Walk us through how a CCM-style encounter actually unfolds: identifying the patient, introducing the program, getting consent, the visit itself, follow-up and escalation to the physician. Help us find where software can remove friction
• Design the guided visit
• Shape the clinical rules
• Reflect state practice differences
• Test and critique
• Inform training and rollout
• Bring the operator's view
• Be a clinical sounding board

Requirements:

• PharmD (or BS Pharm with equivalent experience)
• an active, unrestricted pharmacist license
• 5–12 years in community, retail chain or long-term care pharmacy, including at least 2 years in a leadership role such as pharmacy manager, multi-site or district manager, or clinical services/program coordinator
• Hands-on chronic disease management experience (hypertension, diabetes, hyperlipidemia or similar)
• Experience working under a collaborative practice agreement or protocol with physicians
• Experience launching, scaling or improving at least one pharmacist-delivered clinical service, such as comprehensive medication management, adherence outreach, appointment-based med-sync, immunizations or point-of-care test-and-treat
• Strong comfort with pharmacy software: pharmacy management systems, EHRs, clinical documentation platforms and care-opportunity or gap-in-care tools
• The ability to explain clinical workflows clearly to engineers and designers, and to give direct, constructive feedback
• An active license in Idaho or Florida, or detailed knowledge of their collaborative practice rules (including Florida's collaborative practice certification)
• Experience with Medicare Part B CCM, TCM, MTM or remote patient monitoring programs, including time-based documentation and physician supervision requirements
• Community pharmacy experience in an independent pharmacy cooperative network, a regional or employee-owned chain, or a grocery/retail chain pharmacy
• Experience with value-based care programs: closing gaps in care, health plan quality measures (such as Medicare Star Ratings adherence measures), or pharmacy-side documentation that supports medical billing for clinical services
• Experience in pharmacy informatics, EHR/template build, workflow design, or rolling out new software or standardized workflows across multiple pharmacy locations
• Exposure to rural or multi-site operations, including telepharmacy, central fill or long-term care pharmacy
• Experience working with payer, pharma or technology partners on pharmacy programs
• Board certification (BCACP or BCPS), or ambulatory care residency training
