About the service:
The Mental Health Care Partners Program is embedded in the Caboolture Medicare Mental Health Centre (MMHC). The program supports adults aged 18-65 years of age, who have complex mental health needs, to facilitate improved access to and alignment between primary care and mental health. The multidisciplinary team blends clinical and non-clinical supports to support individuals for up to two years.

Deliver high quality evidence based clinical care as a member of the multidisciplinary team to achieve better health outcomes for adults aged 18-65 years who experience mental health concerns and their families and carers.
