As a Floating Visit Nurse , you'll experience the full scope of home health nursing by working with different patient populations, supporting multiple clinical teams, and caring for patients throughout the Twin Cities Metro . You'll become one of the most versatile nurses on our team while enjoying the stability of a full-time position.

What You'll Be Doing

• Provide skilled home health nursing visits throughout the Twin Cities service area
• Complete comprehensive patient assessments
• Perform wound care , medication management, injections, and other skilled nursing procedures
• Educate patients and caregivers on treatment plans and disease management
• Coordinate care with physicians and interdisciplinary team members
• Provide coverage across territories to support patient needs and growing census
• Complete accurate documentation using Homecare Homebase (HCHB)
• Help ensure continuity of care while delivering exceptional patient experiences

Who We're Looking For

• Active Minnesota Registered Nurse (RN) license
• Experience in home health , hospital, rehabilitation, skilled nursing, or acute care preferred
• Strong assessment and critical thinking skills
• Comfortable adapting to different clinical environments
• Excellent communication and time management abilities
• Reliable transportation with the flexibility to travel throughout the Twin Cities Metro
