Hillcrest Health & Living is looking for a compassionate and motivated Physical Therapist to join our rehabilitation team at Hillcrest Health & Rehab in Bellevue, NE.
Hillcrest Health & Rehab is Bellevue’s only skilled nursing facility, providing short-term rehabilitation, long-term care, and post-acute services in a warm, supportive setting. Our therapy team works closely with nursing, providers, residents, families, and interdisciplinary partners to help individuals regain strength, confidence, and independence.

What You’ll Do
As a Physical Therapist, you will evaluate residents and create individualized treatment plans that improve mobility, reduce pain, restore function, and support independence.
You will use evidence-based treatment techniques while collaborating with the interdisciplinary team to help residents achieve meaningful goals and experience the highest possible quality of life.
You will:
• Complete physical therapy evaluations using appropriate assessments and establish individualized plans of care.
• Provide evidence-based treatment interventions to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Progress treatment plans based on resident needs, clinical findings, goals, and functional outcomes.
• Educate residents and family members on treatment plans, home exercise programs, safety strategies, equipment use, and recommendations for continued care.
• Complete timely and accurate documentation, coding, charging, and required clinical records.
• Collaborate with physicians, nursing, therapy team members, and other interdisciplinary partners to coordinate resident care.
• Assist with discharge planning, equipment recommendations, and transitions between care settings.
• Supervise, collaborate with, and delegate appropriate care to therapy assistants, rehabilitation technicians, and support staff in accordance with applicable regulations.
• Identify residents who may benefit from additional medical or therapy services.
• Manage an efficient resident schedule while considering safety, resident preferences, environmental needs, and productivity expectations.
• Maintain a strong focus on infection control, resident safety, equipment safety, quality outcomes, and regulatory compliance.
• Participate in professional development, clinical education, advocacy, presentations, or other activities that support the therapy profession.

• Strong clinical judgment or the desire to build it through mentorship and experience.
• Ability to stay organized, complete accurate documentation, and manage competing priorities.
• A calm, flexible, and professional approach in changing or stressful situations.
• Strong attention to detail and commitment to safe, high-quality, resident-centered care.
• Comfort using technology and clinical resources efficiently and effectively.
• ABPTS clinical specialty certification in geriatric, orthopedic, or neurological physical therapy welcomed but not required.
Education & Credentials
• Graduate of an accredited Physical Therapy program required.
• Active Physical Therapist license in Nebraska or a compact state required.
• Basic Life Support certification through the American Heart Association required.
