Hillcrest Health & Living is looking for a compassionate and motivated Physical Therapist Assistant to join our rehabilitation team at Hillcrest Health & Rehab in Bellevue, NE.
Hillcrest Health & Rehab is Bellevue’s only skilled nursing facility, providing short-term rehabilitation, long-term care, and post-acute services in a warm, supportive environment. Our in-house therapy team works closely with nursing, providers, social workers, and families to help patients recover after illness, injury, surgery, or hospitalization and return home stronger whenever possible.

What You’ll Do
As a Physical Therapist Assistant, you will carry out individualized treatment plans established by the supervising Physical Therapist. You will use evidence-based techniques to help patients improve mobility, reduce pain, restore function, prevent disability, and work toward meaningful recovery goals.
You will work as part of an interdisciplinary team while communicating regularly with the supervising therapist to adjust treatment approaches and support successful outcomes.
You will:
• Provide patient treatments in accordance with established plans of care and evidence-based guidelines.
• Help patients improve mobility, strength, balance, function, and independence.
• Educate patients and family members using clear communication, adult learning principles, and sensitivity to individual cognitive and cultural needs.
• Complete accurate clinical documentation and charging to support therapy services.
• Collaborate with physicians, nursing, therapy team members, and other interdisciplinary partners to support patient care.
• Communicate regularly with the supervising Physical Therapist regarding patient progress, concerns, goals, and treatment plan needs.
• Assist with transition planning between service lines, including therapy recommendations, equipment needs, and handoff communication.
• Delegate appropriate tasks to therapy technicians and support personnel in accordance with federal and state regulations.
• Build and manage an efficient patient schedule while considering patient preferences, safety, cognitive needs, and productivity expectations.
• Identify patient and facility risks and take appropriate action related to safety, infection control, equipment needs, documentation, and patient handling.
• Assist in identifying patients who may benefit from additional therapy or medical services.
• Participate in professional development, advocacy, clinical education, presentations, or other activities that support the therapy profession.
What We’re Looking For
We are looking for someone who is clinically strong, compassionate, organized, and comfortable working with aging adults in a rehabilitation and skilled nursing setting. The right candidate will be able to build trust with patients and families while staying focused on safety, quality, and measurable progress.
Ideal candidates will have:
• At least two years of experience as a Physical Therapist Assistant in long-term care, post-acute care, hospital, home care, or a similar setting preferred.
• Strong understanding of evidence-based physical therapy treatment techniques.
• Ability to work effectively within an interdisciplinary care team.
• Strong communication skills with patients, families, providers, and team members.
• Ability to manage multiple patient needs while maintaining quality care and accurate documentation.
• A calm, flexible, and professional approach in stressful or changing situations.
• Strong attention to detail and commitment to safety, quality, and patient-centered care.
• Comfort using technology and clinical resources efficiently and effectively.
Education & Credentials
• Graduate of an accredited Physical Therapist Assistant program required.
• Active Physical Therapist Assistant license in Nebraska or a compact state required.
• Basic Life Support CPR certification required or willingness to obtain within 45 days of hire.
