Onboarding and orientation for newly hired RN Care Managers
Develop and maintain standardized onboarding curricula, training materials, job aids, and learning resources
Facilitate virtual and in-person training sessions on RN-CM workflows, care management principles, documentation standards, and technology platforms
Coordinate and oversee shadowing experiences and structured learning plans
Monitor onboarding progress and provide regular feedback to new hires and leaders
Validate readiness for independent practice through competency assessments and skills demonstrations
Develop and maintain RN-CM core competency standards
Conduct annual competency assessments and skills validations
Identify knowledge gaps and create targeted development plans
Support remediation plans for RN-CMs requiring additional coaching or performance support
Ensure consistency in practice standards across markets and regions
Serve as a subject matter expert for RN-CM workflows, care coordination, chronic disease management, care planning, transitions of care, and patient engagement
Monitor onboarding and competency outcomes to identify improvement opportunities
Partner with operational leaders to improve quality metrics, care management outcomes, and documentation accuracy
Requirements
Active Registered Nurse (RN) license in good standing
Bachelor’s degree in Nursing (BSN)
Minimum 3 years of clinical nursing experience
Minimum 2 years of care management, case management, population health, or chronic disease management experience
Demonstrated excellence as an RN Care Manager
Strong presentation, facilitation, coaching, and communication skills
Experience training, mentoring, or precepting staff
Proficiency with electronic medical records and care management platforms
