Serve as a clinical liaison between physicians, patients, families, pharmacies, and external healthcare facilities.
Manage patient communication across telephone and electronic health record systems.
Coordinate multidisciplinary care across physicians, advanced practice providers, patients, caregivers, pharmacies, specialists, and community resources.
Process provider-directed clinical workflows, including medication refills, laboratory and diagnostic orders, referrals.
Complete patient documentation and required forms accurately within the electronic health record (EHR).
Provide patient and family education related to treatment plans, medications, preventive care, and chronic disease management.
Maintain compliance with HIPAA and organizational policies, participate in quality improvement initiatives, and maintain required clinical competencies and certifications.
Requirements
Graduate of an accredited nursing program.
Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with Compact Act.
Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
