Assist clients on the CADI, BI, EW and DD waivers in Hennepin County to access needed waiver and state plan services, as well as needed medical, social, education and other services, regardless of the funding source.
Develop and implement a plan of care (CSSP) that is based on the MNChoices assessment results and within the county approved allocation for the client
Inform clients and/or their legal guardians of service options, and assist clients in identifying potential providers and accessing services
Plan and arrange for services and inform the county waiver contact of the proposed services to be authorized, including provider, service type, amount and frequency for entry into MMIS service agreement
Provide services at the client's residence or other community locations, as specified in the CSSP.
Provide crisis intervention services, as needed, including referrals to meet emergency needs for housing, food, clothing or financial assistance.
Respond to all Vulnerable Adult reports in a timely manner of receipt of report. All responses will be documented and available for Hennepin County to review upon request.
Determine client needs for residential treatment and select appropriate placement homes or facilities.
Monitor client progress towards established service goals and determine termination/continuation of service.
Consult with departmental staff, other county departments and community service professionals to develop and provide appropriate and timely services.
Maintain accurate case records and complete forms for documentation of eligibility, treatment cost reimbursements, service referrals, evaluation of client outcomes and general case management purposes.
Track and provide data used to monitor trends and developments relating to client and program needs, and to monitor agency outcomes.
Track billable time worked, by client, for reimbursement
Requirements
Bachelor's degree in behavioral sciences or related field OR
Bachelor's degree in *any *major AND one year of experience as a social worker, case manager or care coordinator
Must have car, auto insurance and driver's license.
