Responsible for the provision of pharmacist care by working in collaboration with multidisciplinary healthcare teams to ensure quality care, education and research, and patient safety.
Works to promote guidelines and policies and assists in the development of programmatic changes to improve the care provided to patients throughout the health system.
Provides education to patients, providers, students, residents, and others.
Assumes a leadership role in the professions of pharmacy and quality improvement.
Requirements
Must possess current licensure as required by state board where services will be provided: MD: Licensed Pharmacist through Maryland Board of Pharmacy OH: Licensed Pharmacist through Ohio Board of Pharmacy PA: Licensed Pharmacist through Pennsylvania Board of Pharmacy WV: Licensed Pharmacist through West Virginia Board of Pharmacy
Advanced degree (Master of Public Health or other degree)
Residency training OR three (3) years of pharmacist experience
