About this role
Role Overview
Collaborate and inform the case management process to facilitate appropriate healthcare outcomes for members
Provide care coordination, support, and education for members using care management tools and resources
Conduct comprehensive evaluations of member’s needs/eligibility
Recommend approaches to case resolution and/or meeting needs
Identify risk factors and service needs that may impact member outcomes
Coordinate and implement assigned care plan activities and monitor care plan progress
Consult with case managers and other health programs to overcome barriers
Identify and escalate quality of care issues through established channels
Utilize motivation interviewing skills to ensure maximum member engagement
Provide coaching, information, and support to empower members
Requirements
2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focus
Must reside in Newport News or Hampton, Virginia
Must possess reliable transportation and be willing and able to travel up to 75% of the time
