Drive patient retention at Oak Street through implementation of Patient Engagement and Retention workflows
Manage relationship with key partners across provider services, Salesforce and patient comms and marketing
Own proactive workflows related to patient communications and panel reassignment
Contribute to key organizational retention goals through implementing initiatives to meet annual patient wellness visit completion goals
Identify key factors that influence annual wellness visit completion and target patient populations for outreach
Report on success of different engagement strategies for achieving visit completion goals
Project manager for ongoing initiatives related to patient segmentation
Create metrics and reports to monitor success and identify improvement opportunities

Minimum 2-4 years of experience
High level of organization and detail orientation
Experience collaborating on projects across multiple departments
Proficient with Microsoft Excel or other standard analysis tools (SQL, Alteryx, etc.)
Strong intellectual curiosity and problem-solving ability
Quick learner comfortable with ambiguity
Strong accountability and discipline against timelines and deliverables
Collaborative working style, able to work cross functionally
Bachelor’s degree required
