Licensed Practical Nurse – Case Manager

Role Overview

• Conduct routine follow-up assessments in the homes of individuals receiving care.
• Serve as a resource to program staff pertaining to individuals’ medical needs.
• Conduct observational assessments during 90-day home visits.
• Complete required annual training(s) and participate in ongoing professional training and development.
• Report significant events concerning individuals to appropriate parties.
• Participate in staff and regular supervision meetings.

Requirements

• A Licensed Practical Nurse in good standing through the Virginia Board of Nursing is required.
• Previous experience doing home-based visits is preferred but not required.
